Authorization Form for Direct Deposit


Shareholders are encouraged to sign up for direct deposit so Huna Totem Corporation can deposit dividends/distributions directly into their bank accounts. Shareholders may sign up for direct deposit via MyHunaTotem, by completing a Direct Deposit Form and submitting a voided check stub, or by having their bank or financial institution fax account information directly to HTC Shareholder Relations at (907) 789-1896.

Change of Address


If a shareholder has moved or changed the place their mail is delivered, a Change of Address request should be submitted to update his/her address. Shareholders may update their address via MyHunaTotem or by completing a Change of Address Form or written request and submitting it to Shareholder Services via email, fax or mail. Shareholders may also change their address in person at the corporate office or by phone (907) 789-8500. Only a shareholder, power of attorney, legal guardian, or custodian of a minor may change the shareholder’s address.

Testamentary Disposition "Will for Shares"


A Testamentary Disposition is a “Last Will and Testament” for your HTC shares that legally declares who your shares will go to when you pass. You are able to leave your shares with any member of your family, including your spouse. Reminder: The form must be completely filled out, notarized, and returned to Shareholder Relations at Huna Totem to be considered valid. Notary services are offered free of charge at Huna Totem and select banks, just remember to bring your State identification with you. A representative at the U.S. Post Office can also witness your signature. You can update your testamentary at any time. Only the most recently dated testamentary disposition will be considered valid. Request for Name Change


To change your name on stockholder records at Huna Totem Corporation, you will need submit a Request for Name Change. If your name was changed due to legal action, for example: divorce, adoption, marriage, or legal change of name, you must attach a copy of the Court Order authorizing the change. For example: a copy of your divorce decree establishing the right to use your maiden name. Request for Check Reissue


If a check issued to a shareholder is lost or stolen, the shareholder can submit a Check Reissue Form to stop payment and have a new check reissued. Policy: Shareholder checks are valid for 180 days before they become void and must be reissued. The earliest a stop payment will be issued on a check is 30 days after the issue date. Proxy Voting Incentive checks will only be reissued for one year; however, shareholder dividend distribution checks can be reissued for an indefinite period of time. Proxy payments that are not claimed within that time are forfeited to the corporation. It takes approximately 5‐10 business days for a check to be processed and reissued. Huna Totem must pay a fee for every stop payment the bank processes. Therefore, if the check to be reissued in not in hand and a stop payment is required, a $30 reissue fee will be deducted from the check balance. Check Reissue Forms must be complete with all required information, signed, and dated before submission. Huna Totem will not accept Check Reissue Forms that are incomplete, from relatives of the shareholder, or submissions via phone. A Power of Attorney, legal guardian, and custodian of a minor may authorize a Check Reissue Form via the same methods stated above.

Gifting of Shares Packet


Under the terms of ANCSA, shareholders who are at least 18 years of age are able to gift to a child, grandchild, great grandchild, niece, nephew, brother, or sister. Recipients must be related by birth or legal adoption before the recipient turned 18 years of age. By law, the recipient of shares must be Alaska Native as defined under ANCSA. To complete a gifting of shares, the shareholder must submit the Gifting Packet to Shareholder Relations with the required documents for proof of relationship and proof of Alaska Native blood quantum. Under ANCSA amendments, shareholders cannot gift shares to parents, spouses, or relatives by marriage.

Designation of Custodian


This document certifies that the custodian for all stock and other property that the above-named is entitled to receive from Huna Totem Corporation, pursuant to the provisions of the Alaska Native Claims Settlement Act of December 18, 1971, 85 State. 688 (43U.S.C. 1601 et seq.) (ANCSA), determined in accordance with the provisions of Section 13.46.085 of the Alaska Statutes.

UA College Savings Plan Option


As a shareholder you can legally set aside a percentage of your distribution to your UA College Savings Plan account, once you have established one. This form authorizes Huna Totem to transfer the designated percent of your distributions to a UA College Savings Account.

W-9 Tax Form for Huna Totem Corporation stock


One W-9 form is needed for taxable income to the IRS. Once this form is filled out, it is used for the duration of time you are a shareholder.

Estate Questionnaire


An estate questionnaire is submitted to a Shareholder's family when the shareholder passes away without having a valid testamentary disposition on file with the corporation. The Estate Questionnaire assists the shareholder relations department on following Alaska Intestate Succession laws in the estate settlement process.

Burial Assistance for Deceased Shareholders


Applicant must be an immediate family member (mother, father, brother, sister, son or daughter), spouse of the deceased Huna Totem Corporation shareholder or legal representative to be considered eligible to apply for Burial Assistance. If the deceased has no surviving immediate family members or spouse, a letter of confirmation must be provided, and will be reviewed on a case by case basis. Applicant must complete and submit the following within 6 weeks after the date of death: Burial Assistance Application, Death certificate or letter from a medical facility/funeral home with the name of the deceased, and date of death. All applications must be complete in order to be processed. Incomplete applications will not be processed and will be rejected after the 6 week time period. The Burial Assistance Program is based on availability of funds. If the Burial Assistance application is approved by HTC in its sole and absolute discretion, HTC will disperse funds directly to the funeral home or mortuary within a week of the approval date.
